Need urgent advice on securing my data.

I’ve been having constant data breaches/unauthorised access to my accounts for nearly a year now - previous investigation by my bank indicated whoever accessed my accounts had complete access to my username/password/security codes/messages/notifications etc.

I definitely didn’t let anyone else have access to my devices but had my doubts on the internet connection, had been searching for a while and I think I finally have an answer.
