My wife was setting up her Evergreen Health account and was asked privacy questions provided by a 3rd party including one about my dead mothers birthday which they should have no information about.

So my wife was asked to set up a new account for Evergreen Health as they are switching systems. While she was going through the process, they notified her that they would be verifying her information through questions provided by a 3rd party system (possibly One Trust). One of the auto-generated questions asked for my mother's (her mother in law) birthday.
