How do you manage vulnerabilities when dealing with multiple teams in order to remediate them

I’ve found that it has been extremely difficult to track vulnerability statuses when vulnerabilities can’t be fixed by you and rather needs to be sent to a dev team to fix. How do you go about managing each vulnerability and keeping on track which ones have been fixed and which have not. Are there tools you use? Or are you just in constant touch with the individuals assigned to fix the vulnerability? How does this all get recorded?
