Cyber Awareness Maturity Model

Is there a Cyber Awareness maturity model available to assess the maturity level of awareness and Cybersecurity culture of an organization?
I looked into SANS and CIS 18. To my knowledge, SANS does not have any questionnaire/checklist for this and CIS labels 8 out of 9 controls for IG1 and all controls for IG2 & IG3.
