How and when are you choosing to update the IOS software on your Cisco Switches?

I am using a vulnerability management program that performs authenticated scans against several Cisco Catalyst (and other) switches. I've recently discovered several core and branch office switches running IOS versions that have multiple "self-reported" vulnerabilities in the IOS version.
