Account lockouts with no login failures! Whats going on?

Our tools are showing "brute force attack" alerts because thousands of accounts in AD are being locked out. And i'm talking sometimes a dozen lockouts per account per day.
But here's the kicker - we have no login failures. So there are no failed logins at all for 99% of the affected accounts.
