So, for a while I've been brain noodling on how to address this situation addressed. Well, I'll just say a known government consulting firm - that I once worked for - is responsible for entertaining (3) security breaches for a reputable 3LA (i.e., Three-Letter-Agency).
The first breach involved them provisioning my account(s) wrong. Thus, provisioning me under the wrong cleared personnel who shared the same name as myself; as this lasted for 3 months with them not knowing …
