Are penetration testers also creating malware payloads?

I am working in the malware reversering field. However, penetration testing seems in higher demand when looking for jobs. I would like to know if penetration testers are also developing malware when doing penetration tests/red teaming? Not sure exactly what a penetration tester does. If it involves more than initial access.
